Last year, you completed a Pupil Passport for your child, which helped us to better understand their needs, including how they communicate, their likes and dislikes, and other important information to support them in class.

To save having to complete the form again from scratch, this year we are asking you to share any updates using the following two forms:

1. Getting To Know Your Child Form

Please use the form (via the link above or the QR code below) to let us know of any new or updated information. If you would like a copy of your child’s Pupil Passport from 2025/2026, please contact the school office at 501office@hazel-oak.solihull.sch.uk, and we will be happy to send one for you to refer to when completing the form.

2. Photo / Local Trip Consents 

We understand that this is a form you were previously required to complete and renew each year while your child is with us. Going forward, once it has been completed, it will remain valid for the entire duration of your child’s time at Hazel Oak.
